Health Package – Terms & Conditions
These Terms & Conditions ("T&C") govern the purchase and use of health packages offered by EARTHMOON HEALTH CARE SERVICE PRIVATE LIMITED ("Earthmoon", "Company", "we", "us", or "our"). By purchasing, booking, or availing any health package, the customer/patient ("you", "your") agrees to be bound by these T&C.
1. Eligibility & Scope
Health packages are available to individuals as specified in the package description. Packages are valid only at Earthmoon facilities or partner centers mentioned at the time of booking. Packages are meant for preventive or diagnostic purposes unless explicitly stated.
2. Package Inclusions & Exclusions
Inclusions are strictly limited to tests, consultations, procedures, and services listed in the selected package. Any service not expressly mentioned shall be treated as excluded and charged separately. Medicines, consumables, injections, contrast agents, OPD/IPD charges, emergency care, and complications are excluded unless specified.
3. Pricing & Taxes
Prices are as published or quoted at the time of purchase. Applicable taxes such as GST shall be charged as per prevailing laws. Prices may change without prior notice; however, confirmed bookings will be honored at the booked price.
4. Booking, Payment & Confirmation
Full or part payment (as specified) is required to confirm booking. Confirmation is subject to availability of slots, equipment, and partner centers. Valid identification and booking confirmation must be presented at the time of service.
5. Validity, Rescheduling & Cancellation
Package validity is as mentioned in package details and starts from the date of purchase. Rescheduling is allowed subject to availability and prior notice. Cancellations, if permitted, will be processed as per the cancellation policy. No-shows or expiry may result in non-refundability.
6. Refund Policy
Refunds, if applicable, will be processed to the original payment method within standard banking timelines. Partial utilization may not be eligible for a full refund. No refunds will be provided for services already availed.
7. Medical Disclaimers
Health packages do not substitute professional medical diagnosis or treatment unless stated. Test results should be reviewed by qualified medical practitioners. Earthmoon is not responsible for decisions taken solely based on package reports.
8. Patient Responsibilities
Patients must provide accurate medical history and personal information. Fasting or preparation instructions must be followed strictly. Compliance with safety, hygiene, and facility rules is mandatory.
9. Partner Centers & Third Parties
Certain services may be provided through authorized partner centers. Earthmoon facilitates coordination; however, clinical services are rendered by respective providers. Liability rests with the service provider as per applicable laws.
10. Reports & Turnaround Time (TAT)
Report turnaround time is indicative and may vary due to technical or operational reasons. Reports will be shared digitally or physically as specified at booking.
11. Limitation of Liability
Earthmoon’s liability, if any, shall be limited to the amount paid for the package. We are not liable for indirect, incidental, or consequential damages.
12. Force Majeure
Earthmoon shall not be liable for delays or failures caused by events beyond reasonable control, including natural calamities, epidemics, strikes, or government actions.
13. Privacy & Data Protection
Personal and medical data will be handled as per applicable data protection laws and our Privacy Policy. By availing services, you consent to data collection and processing.
14. Amendments
Earthmoon reserves the right to modify these Terms & Conditions at any time without prior notice. Updated terms will be effective upon publication.
15. Governing Law & Jurisdiction
These Terms & Conditions shall be governed by the laws of India. Courts at the Company’s registered office shall have exclusive jurisdiction.
